This Privacy Policy (“Privacy Policy”) explains the ways in which Heuro Inc. and its affiliates and partners (including Heuro Health and Heuro Medical PC of PA) (collectively, “Heuro,” “we,” or “us”) collect, store, use, disclose, and otherwise process personal information through our websites (the “Sites”).
Currently, the Heuro Mobile App (the “Heuro App” or “Application”) is available only to enrolled members of a Heuro plan. Any protected health information processed by the Heuro App is governed by our Notice of Privacy Practices, https://heurohealth.com/notice-of-privacy-practice.
This Privacy Policy applies to Heuro’s processing of “Personal Information,” which means information that identifies or is reasonably identifiable to you or your device.
Please also review our Terms of Use for additional terms and conditions applicable to the Sites.
We collect several types of Personal Information through the Sites:
Identifiers and contact information (e.g., name, address, email address, account names)
Commercial and transactional information
Internet or other network or device activity (e.g., IP address and areas of the Sites that you visit)
General geolocation information (e.g., your city and state or zip code)
Customer service data (e.g., information you provide through a chat, SMS or call to our customer support team)
Login credentials
Heuro collects information in the following ways:
Information you give us. When you ask us for more information about Heuro’s services, we collect your name, email address, phone number, and other contact information.
Information we collect automatically. We collect information about the Services that you use and how you use them. This information includes:
Computer, tablet, or mobile telephone information. We may collect device-specific information (such as your hardware model, operating system, unique device identifiers, device sensors and mobile network information including phone number) and device sensors and related device hardware. Heuro may associate your device identifiers or phone number with your Heuro Account.
Computer, tablet, or mobile telephone information. When you use the Services or view content provided by Heuro, we automatically collect and store certain information in server logs, including:
Internet protocol address.
Device information such hardware settings, duration of use
Site navigation data, system activity, and device crashes.
Information collected from the Sites including platform type, the number of clicks, files you download, domain names, landing pages, pages viewed and the order of those pages, the amount of time spent on particular pages, the terms you use in searches on our sites, the date and time you used the Sites, error logs, and other similar information.
Cookies and similar technologies. Cookies are small text files that are stored on your device. We use cookies and similar means to store information to customize your experiences and improve the functionality of the Sites.
Information obtained from other sources. We may obtain information about you from affiliates, partners, and other third-parties. This information may be used to provide services to you and to provide analysis about you in comparison to people who are demographically similar to you. We may combine the information we obtain from third- parties with information that we have collected about you.
To provide Services to you. We and our Service Providers use Personal Information to provide, maintain, protect and improve the Sites.
For the conduct of our Business Operations, Product improvement and research, Personalization, Customer support, and Communications.
To train artificial intelligence and/or machine learning models that support the Sites.
For Marketing and Advertising (e.g., to send you emails about our products).
To personalize your experience on the Sites.
To provide customer support and respond to your questions.
To send you information, including confirmations, technical notices, updates, security alerts, reminders, support, and administrative messages.
To create de-identified information. We may aggregate, anonymize, or de-identify Personal Information. This Privacy Policy does not apply to our use or disclosure of de-identified information.
We may use AI based models to process data in order to provide tailored responses. Responses generated by the AI are specifically based on the information provided during your use of the Services. We may retain the data provided by you with AI dialogues for use in the future and provide better services to you.
Heuro collects information in the following ways:
With your consent. We may share your Personal Information with companies, organizations or individuals outside of Heuro when we have your consent to do so.
Third-party service providers. Heuro does not sell or rent Personal Information. We may disclose your information with our third-party service providers that process Personal Information on our behalf and at our direction.
For external services. We provide Personal Information to our affiliates or other trusted businesses or persons who provide services to you or us, based on our instructions and in compliance with our Privacy Policy and any other appropriate confidentiality and security measures.
We may disclose the same as part of a corporate transaction or proceeding such as a merger, financing, acquisition, bankruptcy, dissolution, or a transfer, divestiture, or sale of all or a portion of our business or assets.
For legal reasons. We will share Personal Information with companies, organizations or individuals outside of Heuro if we have a good-faith belief that access, use, preservation or disclosure of the information is reasonably necessary to:
Meet any applicable law, regulation, legal process or enforceable governmental request;
Enforce applicable Terms of Use, including investigation of potential violations.
Detect, prevent, or otherwise address fraud, security or technical issues.
In response to a court order, judicial or other government subpoena or warrant, or to otherwise cooperate with law enforcement or other governmental agencies.
Protect against harm to the rights, property or safety of Heuro, our users, or the public as required or permitted by law.
We may share aggregated, non-personally identifiable information publicly and with our partners to evaluate the effectiveness, value, and analytic trends of the Services.
Other services: If you link your account or direct us to share Personal Information, that data is governed by their privacy policies.
Heuro is committed to protecting Personal Information, and we use a variety of technical, administrative, and procedural data security measures. However, no method of transmission over the Internet, and no means of electronic or physical storage, is absolutely secure. As such, we cannot guarantee the security of your information transmitted to, through, or on our Services or via the Internet and that any such transmission is at your own risk.
Updating or correcting Personal Information. Our goal is to give users ways to update and correct their information quickly or, alternatively, delete the information, unless we need to keep the information for legitimate business or legal purposes. When updating your Personal Information, we may ask you to verify your identity before we can act on your request. Where we can provide information access and correction, we will do so for free, except where it would require a disproportionate effort. We aim to maintain our Services in a manner that protects information from accidental or malicious destruction. Because of this, after you delete information from our Services, we may not immediately delete residual copies from our active servers and may not remove information from our backup systems. Access, correction, or deletion requests can be made by contacting Heuro Customer Support at support@heurohealth.com.
Account Closures. Upon termination (by you or by Heuro) of a relationship with a Service Provider supporting a Sponsored Program (e.g. termination of your employment or cessation of the program), you will no longer have access to your Heuro Account or the Personal Information generated during the Sponsored Program.
Providing Access - Access to the information that we have collected about you and a copy of certain limited set of information in a portable format.
Right Against Discrimination. We will not discriminate against you for exercising these rights. You may designate through a valid power of attorney another person to exercise these rights on your behalf.
Information of Minors. The Services are not directed to or intended for use by anyone under the age of 18. In compliance with laws, we will not intentionally collect any Personal Information from minors under the age of 18. If you think that we have collected Personal Information from a minor, please contact us by e-mailing privacy@heurohealth.com. We will take reasonable steps to delete that information promptly.
California Do Not Track Notice. Some browsers have a “Do Not Track” feature that lets you tell websites and online services that you do not want to have your online activities tracked. Our websites and Services do not respond to “Do Not Track” signals.
Our Privacy Policy may be amended from time to time. This Privacy Policy is not intended to and does not create any contractual or other legal rights in or on behalf of any party. If we change this Privacy Policy, we will post changes on this page and, if the changes are material, we will provide you with notice as required by applicable law.
